2006 to August 31, 2007) for
- Most prescriptions drugs or medicines
- diabetic supplies and insulin
- prescribed hair-loss remedies
- some vaccines
- prescribed smoking cessation aids (up to
$50.00 per benefit year)
- oral contraceptives excluding "patch"
Exclusion: - all “over the counter” products
- allergy serums
- vitamin treatments
- Diane 35*
- fertility drugs
- external contraceptives (injections)
*physician’s referral required
